If you have the rank of Lt. Commander, you will still be referred as Commander.
Why? Is it just because it takes too much time or what?

Here is another idea, full title is obvious for introduction, but is not needed for commands because of using persons name, Worf or Cmd Worf, and yes it is shorter. The only time I could see full title when giving commands is if you have two officers of different rank in the same area with the same name, primarily with temperal incursions (a person either jumps ahead or back in time when he/she is a different rank).

I've often wondered though, what would it be like to be the president of the Federation.

Crewman isn't after cadet. Cadet's who graduate are promoted to Ensign. A person is called a crewman or midshipman if they are a non-commissioned officer. ie didn't spend four years at the Academy.

Um...you forgot Lieutenant Junior Grade, Lower Rear Admiral (one pip surrounded by a box) Upper Rear Admiral (two pips in a box), Vice Admiral (three in a box) and also they got rid of the title Commodore. After Captain would be based on merit as to whether you become a lower or upper Rear Admiral or skip that and go straight to Vice Admiral.
And I don't think crewmen are above a cadet. Just like the US Navy, officers and officers in training (i.e. cadets) are still higher rank than enlisted men and women. For example, Chief O'Brien may have served Starfleet for twenty years but he still has to take direct orders from an Ensign who has just graduated the Academy. Usually, though, the officers understand the value of enlisted men as the ones who actually do the work and know the systems they specialize in. Officers just tell enlisted when to do it.
