Tuvok's evil twin brother, Threevok, was originally going to be cast for his part, until he was reduced to Onethirdvok in a tragic math accident.

In a scene that was filmed but never used for the episode "Tinker, Tenor, Doctor, Spy", Doc combines the Photonic Cannon with Seven of Nine in an... interesting way.

B'Ellana knocks on Chakotay's door in the middle of the night. "Dammit Bell, it's the middle of the night... what is it?" "How come Seven's Chief Engineer and I'm not?" Chakotay stands a little straighter. "Well, ah... you know... she's just been doing a really bang-on good job..." There was a little sound from Chakotay's quarters. "Chakotay... are you going out for more alcoholic beverages? I am experiencing what my research says is a 'hangover'..."

Genavieve Bujold, who was originally going to be cast as Janeway, was unable to continue work because she killed the cameraman, director, and left several of the other actors in a coma because she bored them to death with her voice. She was subsequently arrested for unintentional murder, and they made sure Kate Mulgrew was able to display emotion before casting her.
