Much like with normal technology, each piece of equipment has been designed for a specific purpose, with the ability to expand upon that purpose. The standard issue Tricorder is able to pick up a lot of environmental, geological, etc. etc., while the Medical Tricorder is designed souly for medicinal purposes, it cannot scan the environment, it cannot detect abnormal things etc. etc., but it's medical database is expanded to cover for it.

Actually the normal tricorder has medical features
but just for discerning whats wrong
The medical tricorder also has a medical scanning peice
its connects to the tricorder and feeds it more specific information.
